Situating Pediatric Pain In the United States, pediatric pain medicine is located in the realm of tertiary care, medical services performed by physicians with specialized training.

These terms all point, in various ways, to the failures of biomedicine: failures to explain adequately why pain has occurred and to respond adequately to its presence. Certainty is the second dimension of biomedical explanation identified previously. While uncertainty is endemic across medical settings, the uncertainty inherent in pain explanations is of a very particular sort. Whereas much medical uncertainty crystallizes around epistemological questions—for example, how physicians can know what will prove the best course of treatment—pain clinicians must grapple with ontological uncertainty: that is, uncertainty about whether a person is truly in pain and the nature of pain itself.

In one notable case, Kaiser Permanente, an HMO insurance company,90 approved eight sessions with Charlotte LeFevre because there were no pediatric pain clinicians in its network. However, this approval was only granted following the persistent advocacy of the patient’s mother over a twomonth period and a thick stack of paperwork completed by Nina and Charlotte. Cases like this proved the exception rather than the rule. The clinicians worked to find creative solutions for families in need, lowering fees, tapping into free resources such as school counselors and university research studies that might offer short-term psychotherapeutic services free of charge, and, when necessary, privileging physical therapy and acupuncture, which held some hope of insurance reimbursement, over other therapeutic modalities.
